Case Management & Social Services

Orchard Hospital's Social Services Department can assist in coordinating services with your physician, family members, and community agencies.

We offer many services to help you manage your condition and treatment.  One of which is Discharge Planning.  Discharge planning assures continuity of care after you leave the hospital. Discharge planning begins when you are admitted to the hospital and is coordinated with your doctor's plan for treatment.

Your health care team will talk with you and your family about options for discharge. A social worker may meet with you and your family to assess your needs and identify available resources. This assessment addresses your:

  • Physical and emotional needs
  • Available assistance from family and friends
  • Community resources
  • Financial resources

Following assessment, your social worker will discuss options and recommend the most appropriate plan. When you and your family decide on a plan, your social worker will coordinate the arrangements.

We are also happy to assist in coordinating the following services or directing you to the appropriate agency for additional assistance:

  • Medical equipment needs
  • Rehabilitation
  • Hospice services
  • Referrals for the behavioral therapy and support groups

Orchard Hospital is a SAFE SURRENDER SITE: By state law, an infant up to 72 hours old may be left with a health care profession at Orchard Hospital, legally and anonymously.
